DATA SUBJECT APPLICATION FORM AND GENERAL EXPLANATIONS
Personal data owners ("Data Subject") have the right to apply to Düş Yeri Bilişim Teknoloji ve Animasyon A.Ş. ("Company"), the data controller, in order to exercise the rights set out under Article 11 of Law No. 6698 on the Protection of Personal Data ("KVKK"). The rights of the Data Subject are as follows:
- To learn whether your personal data is processed.
- To request information if it has been processed.
- To learn the purpose of processing your personal data and whether it is used in accordance with its purpose.
- To know the third parties to whom it is transferred domestically or abroad.
- To request the correction of personal data in the event that it has been processed incompletely or incorrectly.
- To request the erasure or destruction of your personal data within the framework of the conditions set out in the KVKK.
- To request notification of the transactions carried out pursuant to your rights of correction, erasure and destruction mentioned above to the third parties to whom the personal data has been transferred.
- To object to the emergence of a result against you by analysing your processed personal data exclusively through automated systems.
- To request compensation for damages in the event that you suffer damage due to the processing of your personal data contrary to the relevant legislation.
1. EXPLANATIONS REGARDING THE APPLICATION METHOD:
In order for our Company to respond to applications made by Data Subjects under Article 11 of the KVKK, the application procedure must comply with the provisions of the Communiqué on the Procedures and Principles for Application to the Data Controller ("Communiqué"). Our Company has prepared this application form in order to facilitate applications being made in the manner prescribed by legislation. It should be emphasised that, pursuant to paragraph 2 of Article 4 of the Communiqué, Data Subjects may benefit from this right provided that they make their applications in Turkish.

* Data Subjects who apply by way of Written Application in Person are obliged to present documents verifying their identity. In this context, for written applications you make in person, we kindly ask that only the front side of a photocopy of your identity card (in a manner that does not show blood type and religion fields) be presented together with the application form.
** We kindly ask that "Personal Data Protection Law Data Subject Request" be written on the notification envelope of the application form or in the subject line of the e-mail.

Our Company always reserves the right to request additional information and documents verifying your identity, in order to prevent the unlawful sharing of your personal data with third parties and to ensure the security of your personal data.
Data subject requests will be concluded by our Company pursuant to Article 13/2 of the KVKK as soon as possible and within a maximum of 30 (thirty) days depending on the nature of the request.
